CAR-T Therapy in China: World's First Solid-Tumor CAR-T Treatment

China approved the world's first CAR-T therapy for a solid tumor in 2026. Learn how a New Zealand cancer patient travelled to Shanghai for treatment and who may be eligible for satri-cel (CT041).

For years, CAR-T cell therapy has transformed the treatment of certain blood cancers. But applying the same technology successfully to solid tumors has been one of the biggest challenges in cancer medicine.

In June 2026, that changed.

China approved satricabtagene autoleucel (satri-cel, CT041), a CAR-T therapy targeting Claudin18.2 (CLDN18.2). It became the world's first CAR-T cell therapy approved for the treatment of a solid tumor.

Just weeks later, a 59-year-old patient from New Zealand travelled to Shanghai to receive the newly approved treatment.

His case provides an important real-world example of another reason international patients may consider cancer treatment in China: not only potential differences in cost, but also access to certain newly approved therapies.

What Is Satri-cel (CT041)?

Satri-cel is an autologous CAR-T cell therapy developed to target Claudin18.2, a protein that can be expressed by some gastrointestinal cancers.

CAR-T treatment begins by collecting a patient's own T cells. These immune cells are then genetically engineered to recognize a particular target on cancer cells, expanded and subsequently returned to the patient.

CAR-T therapies have already been approved for several blood cancers. Solid tumors, however, have proved substantially more difficult to treat with CAR-T technology.

The approval of satri-cel therefore represents an important milestone in cellular cancer therapy. Nature Biotechnology described it as the first CAR-T therapy approved for solid tumors.

However, the approval does not mean that satri-cel can be used for every solid tumor.

In China, it is approved for patients with CLDN18.2-positive, HER2-negative advanced gastric or gastroesophageal junction adenocarcinoma who have failed at least two previous lines of therapy.

Whether a patient is suitable therefore depends on the cancer diagnosis, previous treatment history and biomarker results.

A New Zealand Patient Travels to China for CAR-T Therapy

One of the first international patients to access the newly approved treatment was Josh, a 59-year-old man from New Zealand.

According to Jiahui International Cancer Center in Shanghai, Josh had been diagnosed with advanced gastroduodenal adenocarcinoma.

He had already received chemotherapy combined with immunotherapy in New Zealand, but his cancer continued to progress.

With conventional treatment options becoming increasingly limited, he began looking internationally for other possibilities.

His search eventually led him to China.

Satri-cel had just received regulatory approval, and Jiahui International Cancer Center was among the institutions preparing to provide the treatment.

Before travelling, Josh underwent biomarker testing in New Zealand.

The testing showed high Claudin18.2 expression and HER2 negativity, which supported his potential eligibility for satri-cel treatment.

He then travelled with his wife to Shanghai.

The CAR-T Treatment Process in Shanghai

On July 6, 2026, Josh underwent leukapheresis at Jiahui International Cancer Center.

Leukapheresis is an early step in the CAR-T treatment process. T cells are collected from the patient's blood and subsequently sent for manufacturing, where they are engineered and expanded before being returned to the patient.

Before cell collection, Josh underwent further evaluation in Shanghai, including PET/CT imaging, multidisciplinary assessment and treatment planning.

The hospital later reported that he proceeded to CAR-T cell infusion and remained in Shanghai for treatment and monitoring.

His overall treatment journey at the hospital lasted more than a month.

His Condition Before CAR-T Treatment

Josh was already seriously ill when he arrived for treatment.

According to the treating team at Jiahui International Cancer Center, he had significant malignant ascites and severe difficulty eating.

Before CAR-T infusion, his abdominal fluid drainage was reportedly around 1,000 mL per day, at times exceeding 2,000 mL per day.

This is important context.

Josh was not travelling to China for early-stage cancer treatment. His disease had continued progressing despite previous systemic treatment, and his clinical condition was already complicated.

What Happened After the CAR-T Infusion?

According to Jiahui's account of the case, Josh's malignant ascites decreased noticeably by approximately the 10th day after CAR-T infusion.

His peritoneal drainage catheter was eventually removed before discharge.

After more than a month of treatment, Josh was discharged from Jiahui International Hospital on August 16, 2026.

Before he returned to New Zealand, his medical team prepared follow-up recommendations and transferred his treatment records to his physician at home.

The Shanghai medical team also held a call with his New Zealand doctor to discuss his treatment and subsequent care.

This kind of coordination is particularly important for international cancer patients because CAR-T treatment does not necessarily end when the patient leaves the hospital.

Ongoing monitoring and oncology follow-up remain important after returning home.

Does This Mean the CAR-T Therapy Cured His Cancer?

No such conclusion should be drawn from this case.

Josh's improvement and discharge are encouraging, but successful discharge is not the same as a cancer cure.

The hospital reported improvement in malignant ascites and his clinical condition following treatment. Longer-term imaging, disease response and follow-up would be needed to understand the durability of that response.

This distinction is particularly important when discussing newly approved cancer therapies.

One individual case can illustrate how a treatment is being used in clinical practice, but it cannot establish how every patient will respond.

What Does the Research Say About Satri-cel?

Satri-cel did not reach approval based on an individual case.

Clinical development of the therapy has been underway for years.

A Phase 1 study published in Nature Medicine evaluated satri-cel in patients with CLDN18.2-positive advanced gastrointestinal cancers.

The subsequent randomized Phase 2 CT041-ST-01 trial evaluated satri-cel in previously treated advanced gastric or gastroesophageal junction cancer. The study formed an important part of the evidence behind the therapy's development and approval.

The June 2026 approval therefore represents the transition of solid-tumor CAR-T from an experimental concept into an approved treatment for a specific group of eligible patients.

It does not mean CAR-T has become a general treatment for solid tumors.

Who May Be Eligible for This CAR-T Therapy in China?

The current approved indication is narrow.

Satri-cel is intended for certain patients with:

advanced gastric or gastroesophageal junction adenocarcinoma

who are:

CLDN18.2 positive,

HER2 negative,

and have:

failed at least two previous lines of treatment.

This means someone with lung cancer, liver cancer, breast cancer or another solid tumor should not assume that this particular approved CAR-T therapy is available for their cancer.

Even among gastric cancer patients, biomarker testing and specialist assessment are necessary.

Josh's case demonstrates this clearly: biomarker testing was performed before he travelled to China, helping establish whether the therapy could potentially match his cancer.

Why Are International Cancer Patients Looking at China?

For many years, international medical travel for advanced cancer treatment was associated primarily with destinations such as the United States, Germany, Japan and Singapore.

China is increasingly becoming part of that conversation.

Cost can certainly be one factor. As we discussed in our guide to cancer treatment costs in China, some conventional cancer treatments can cost substantially less than private-pay treatment in certain Western countries.

But Josh's case highlights a different reason:

access.

In this situation, an innovative cancer therapy became commercially available in China before equivalent approved solid-tumor CAR-T treatments were widely available internationally.

That creates a new type of medical traveller — a patient who is not necessarily travelling abroad primarily to save money, but to investigate a treatment that may not yet be accessible at home.

The Challenges of Travelling to China for Advanced Cancer Treatment

Accessing an advanced therapy in another country is considerably more complicated than simply booking a hospital appointment.

International patients may need to coordinate:

  • medical records and pathology reports

  • biomarker or molecular testing

  • specialist assessment

  • hospital acceptance

  • treatment scheduling

  • travel arrangements

  • accommodation

  • language support

  • local transportation

  • communication with doctors

  • post-treatment follow-up in their home country

For seriously ill cancer patients, some of these logistical issues can become significant.

There is also another important consideration: not every patient who is interested in an innovative therapy will qualify for it.

Eligibility depends on the diagnosis, biomarkers, previous treatments, overall medical condition and the treating physician's assessment.

For that reason, international patients considering treatment in China should first determine whether an appropriate Chinese medical institution can evaluate their case and whether the treatment they are seeking is actually relevant to their condition.

Exploring Advanced Cancer Treatment in China

China's oncology landscape is changing quickly.

The approval of the world's first CAR-T therapy for a solid tumor is one example of how some cancer treatments developed or approved in China may increasingly attract international patients.

However, choosing to travel abroad for advanced cancer treatment is a major medical and logistical decision.
